Agency Meeting

is the final phase of certification, and a great opportunity to have your questions answered by your local respite agency representative(s).

Checklist of items that will be addressed during the meeting:

  • 2 Current Personal References
  • CPR/First Aid Cards (or have a training class scheduled)
  • Background Check (DHHS, Maine DMV and Maine State Bureau of Investigation)
  • W9 Form (Independent Contractor)
  • Vendor Agreement - Provided and Signed at meeting

* Please note that in Region III, the Agency Meeting may take place via telephone and mail.
