How To Become Certified

Getting Certified Is Easy.  Follow these steps
 1   Familiarize yourself with the basic Requirements

* Please note that Region II requires an Agency Meeting or the online equivalent.

 Checklist of items that will be addressed during the meeting:

  • 2 Current Personal References
  • CPR/First Aid Cards (or have a training class scheduled)
  • Background Check (DHHS, Maine DMV and Maine State Bureau of Investigation)
  • W9 Form (Independent Contractor)
  • Provider Agreement - Provided and Signed at meeting
